On-site Workplace Screenings: Our Biometric Screenings provide on the spot results to immediately identify high risk candidates. Results include:

- Pulse
- Waist/Hip Ratio
- BMI/Body Fat Analysis
- Fasting Blood Sugar/Blood Glucose
- Total Cholesterol: HDL, LDL and Triglyceride Levels
Disease Management Program: With our superior clinical and claims management data system, our Disease Management Program Care Coordination Plus provides monthly updates of claims information to our Disease Management Nurses. This in turn allows our nurses to concisely manage high risk employees, providing improved quality of care while decreasing opportunity for a high dollar claim catastrophic event.
On-site Wellness Education Programs: CHP will come to your company to educate your employees on a vast array of wellness education topics. We custom fit our programs to meet your needs, so we can reach your entire employee population - from CEO to the secretary!
On-site Nutrition/Weight Management/Lifestyle Coaching: Our health team will come to your company to provide these services either in a group or in an individual setting. We get the desired results because our local Wellness Care Team takes into consideration all aspects of the person. From the single mom who has "no time to exercise" to the traveling executive "eating on the road", Their health coach will design a workable plan putting the employee on the road to success.
