We start with data. We use data to determine where the problems are. You can’t fix something until you know what’s broken. We extract data from as many sources as possible to create a baseline for each employee. Any data repository can include medical and Rx data, but to get the most complete picture of your employees' health, we also take data from HRA’s, biometric screenings, LTD, STD, workers comp and on-site clinics, if available.
We create an action plan. A major step in reducing trend involves education and engagement. Your employees are most likely in their current situation due to a lack of knowledge and support. They need to be continuously educated and encouraged regarding the lifestyle choices they are making.
We provide on-site coaching. We’ve found that there are some people that respond to telephonic coaching, but it doesn’t engage the majority of the employee population. Our experience shows that providing a local clinical manager to meet regularly with employees on a face to face basis increases the success of disease management programs tremendously.
We track your employee’s success. Since many of the issues your employee's have are chronic diseases, tracking each employee's progress is an ongoing process. Chronic conditions tend to affect 20% of your population, but they typically make up 80% of your claim costs. By definition, chronic conditions aren’t usually cured, but with proper management and lifestyle changes, flare ups can be controlled, reducing health care costs significantly.
