For over 16 years, Community Health Partners, Inc. (CHP) has played an active part in improving the health of the residents in the Upstate of South Carolina.  Essentially, “we are local people taking care of local people”.

Through our collaborative efforts with area employers, Upstate hospitals and local physicians, we all agreed there was a critical need for local, hands on disease management and wellness intervention.  We also determined that measuring the effect of the programs, along with demonstrating a strong ROI, would be essential to its success.
